LAST DAY FOR ORDERS TO BE COMPLETED IN TIME FOR FIELD DAY IS MAY 5TH.

Please do not go by the date of completion given by the website as this is a pre-order for a school event. If shirt is ordered by May 5th you will receive your shirt before May 14th.

Please select your correct grade as each grade will be represented by a different color shirt.

Also include STUDENT NAME and Grade in the Box Provided to allow for easy delivery.

At checkout please choose delivery as method of delivery for your shirt to be delivered to CMS. IF USPS is chosen the shirt will be mailed to your address provided. If pickup is chosen it will packaged and set out for pickup from a provided address in Columbiana.
